• Provide personal care to patients in a manner conducive to their safety and comfort.
• Assist in the provision of restorative or rehabilitative care.
• Perform restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.
• Promote and support the greatest possible degree of independence for patients.
• Advises Director of Rehab (DOR) of patient's status daily.
• Contribute knowledge of patient's conditions/needs to patient care plans.
• Assist in the implementation of care plans as directed within the parameters of established policy and procedure.
• Observe and report on patient's conditions and document observations as required.

• State certification as a Nursing Assistant is mandatory.
• Ability to read, write, and speak the English language.
• Must be available to work six days per week.
• Ability to travel to additional SNF locations within geographical area.
