Hospice Registered Nurse / RN Case Manager

We are seeking a qualified Hospice Registered Nurse / RN Case Manager to provide compassionate care and case management for patients in a hospice setting. The role focuses on enabling patients to spend quality time, building trusting relationships, empowering caregivers, and collaborating with the care team to provide comfort, peace, and dignity.

• Enable patients to spend quality time with loved ones and enjoy activities they value.

• RN license in the state where you work.
• Minimum two years of experience as a registered nurse; hospice experience preferred.
• Valid driver's license.
• Current CPR and BCLS certification.
