Optum's At Home Hospice is seeking a Registered Nurse Case Manager (PRN) to deliver and continually evaluate the Plan of Care for hospice patients.
You will collaborate with the IDG, implement evidence-based strategies, and support families and patients through physical, psychosocial, and spiritual care.
Required:
Texas RN license, 1+ year experience, valid driver's license or reliable transportation; pay $40.20–$60.25 hourly plus comprehensive benefits.
