Private Duty Nurse LVN Part Time

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients’ homes, supervised by a Registered Nurse.

• Delivering and coordinating quality patient care according to physician orders
• Assessing patient condition, monitoring changes, and reporting as needed
• Documenting assessments, nursing actions, and care activities in a timely manner
• Implementing and updating care plans, recognizing patient needs, and preventing risks
• Educating patients and families on disease management, self-care, and prevention
• Ensuring infection control and maintaining confidentiality

• Current, active LPN/LVN license in the practice state
• Current CPR certification
• TB test within the last 12 months
• Preferred: six months of recent clinical experience or home health background
• Strong attention to detail, organization, communication, and problem-solving skills
• Physical ability to lift 50 pounds, reposition patients, and perform home care tasks
• Reliable transportation and ability to travel
• Providing compassionate care while adhering to policies and regulations
• Maintaining professional standards and documentation compliance
• Participating in ongoing education and skill development
• Adapting to varied home environments and potential exposure to health hazards
