SummaryWe are seeking a highly skilled and compassionate Telehealth RN Case Manager to join our dynamic healthcare team. The ideal candidate will be responsible for coordinating and managing patient care remotely, utilizing telehealth platforms to provide efficient and effective healthcare services. As a Telehealth RN Case Manager, you will work closely with patients, healthcare providers, and other stakeholders to ensure continuity of care, promote patient education, and facilitate positive health outcomes.
This role requires strong clinical knowledge, excellent communication skills, and the ability to leverage technology to deliver exceptional patient care remotely.
- Conduct initial patient assessments and develop individualized care plans based on clinical findings and patient needs.
- Travel anywhere within the United States, as needed, to visit doctors’ offices for 2–3 days at a time to complete in‑office assessments.
- Collaborate with healthcare providers, including physicians, specialists, and allied health professionals, to ensure comprehensive care delivery.
- Assess and address patient concerns, escalating issues as needed to ensure timely resolution.
- Educate patients and caregivers on health management strategies, medications, and self‑care techniques.
- Document all patient interactions, assessments, and care plans accurately and timely within electronic health records (EHR) systems.
- Coordinate referrals to community resources and support services as necessary.
- Stay informed about current healthcare trends, telehealth regulations, and best practices in case management.
- Current unrestricted RN license in Michigan.
- BLS certification required.
- Minimum 2 years of clinical nursing experience, with a background in case management preferred.
- Certification in Case Management (CCM) preferred.
- Proficiency in using telehealth platforms and electronic health record (EHR) systems.
- Excellent communication skills, both verbal and written.
- Strong critical thinking and problem-solving abilities.
- Ability to work independently and as part of a multidisciplinary team.

- Flexibility to adapt to changing healthcare environments and patient needs.
