Back Part‑time RN Case Manager – Royal Oak, MI

Walker Healthforce is seeking a RN Case Manager with minimum 2 years of clinical experience to support a health insurance company in Michigan. This is a 12‑month contract opportunity.

• Three part‑time RNs needed:
  • One available Monday, Tuesday, and Wednesday
  • One available Wednesday, Thursday, and Friday
  • One on a rotating schedule – week 1 (Saturday & Sunday), week 2 (Monday & Tuesday)

• Current unrestricted RN license in Michigan
• Minimum 2 years of clinical nursing experience, case‑management experience preferred
• Certification in Case Management (CCM) preferred
• Proficiency using telehealth platforms and electronic health record (EHR) systems
• Excellent verbal and written communication skills
• Strong critical thinking and problem‑solving abilities
• Ability to work independently and as part of a multidisciplinary team
• Compassionate and empathetic approach to patient care
• Flexibility to adapt to changing healthcare environments and patient needs
• Commitment to maintaining patient confidentiality and privacy
• Bilingual:
  Russian, Spanish, Arabic – Preferred

• Conduct initial patient assessments and develop individualized care plans
• Manage a high‑volume caseload of approximately 200 Medicare members while meeting performance standards
• Utilize telehealth platforms to monitor progress, provide education, and coordinate interventions
• Collaborate with physicians, specialists, and allied health professionals for comprehensive care delivery
• Assess and address patient concerns, escalating as needed for timely resolution
• Educate patients and caregivers on health management strategies, medications, and self‑care techniques
• Document all interactions, assessments, and care plans accurately and timely in EHR systems
• Coordinate referrals to community resources and support services as necessary
• Stay informed about healthcare trends, telehealth regulations, and best practices in case management
