We are seeking a highly skilled and compassionate RN Case Manager to join our dynamic healthcare team by the next quarter. The ideal candidate will be responsible for coordinating and managing patient care remotely, utilizing telehealth platforms to provide efficient and effective healthcare services. As a Telehealth RN Case Manager, you will work closely with patients, healthcare providers, and other stakeholders to ensure continuity of care, promote patient education, and facilitate positive health outcomes.

This role requires strong clinical knowledge, excellent communication skills, and the ability to leverage technology to deliver exceptional patient care remotely.
