- Work in conjunction with the Director of Nursing in providing direct supervision of the home care team.
- Be responsible for the delivery of quality patient care including coordination, implementation, and evaluation of home care services as evidenced by positive outcomes.
- Assume 24‑hour accountability for patient care functions and direction of the patient care team.
- Contribute to the attainment of agency goals by assisting in the development of the operational plan and budget.
- Ensure agency compliance with local, state, federal, government regulations and ACHC standards.
- Act as a liaison in the community, articulating and reinforcing the Holy Redeemer Home Care and Hospice story and the value of our service.
- Develop and maintain collaborative relationships with internal and external customers including employees, physicians, facilities, and community members.

RecruitmentRequirements
- Graduate from an accredited school of nursing. BSN and/or degree in a health‑related field preferred.
- Two years of clinical experience required.
- Three to five years of home care experience preferred.
- Current supervisory experience preferred.
- Ability to function in a rapidly changing health care environment.
- Excellent communication skills, customer service skills, and a positive leadership style.
- Computer literacy required.
- Able to travel throughout all HRHS entities.
- Able to travel to patients’ homes within the service area.
Current licensure in the state of practice, valid driver’s license, and current CPR certification.
