Graduate of an accredited school and currently licensed to practice professionally in the state of Louisiana.

Minimum of one year experience as a Physical Therapist; home health experience is a plus.

• Knowledgeable of principles and practices of physical therapy
• Ability to manage patients with different types of personalities
• Excellent observation and assessment skills
• Excellent written and verbal communication skills
• Current CPR Certification
• Current knowledge of treatment practices per discipline and drive to continually learn and grow in the profession.

• Provides physical therapy care to patients in their place of residence.
• Select and adapt the appropriate plan of care, per case
• Partners with patients and their caregivers to develop a rehabilitation goal aimed at reaching the patient’s highest level of functional ability.
• Teaches the patient/family disease process and self-care needs.
• Contributes to the provision of quality care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care
• Collect data and document processes followed
• Promote patients' healthy lifestyle by improving strength, flexibility, balance, and coordination.
