Job Overview
BAYADA Home Health Care is seeking a Home Health Physical Therapist for a position in Rutland, Vermont. The role requires a current Physical Therapist license in Vermont and a graduation from an APTA‑approved program. New PT grads are welcomed. The PT works with clients in their homes, crafting treatment plans and applying innovative techniques, and is part of a multidisciplinary team providing skilled nursing and rehabilitative care.
HowYou'll Make an Impact
- Perform diagnostic tests and measurements (mobility/range of joints, transfer status, stability, ambulation patterns, strength, endurance, balance, safety assessments)
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance work, and other interventions
- Implement integumentary protective interventions to prevent wound formation and promote repair using evidence‑based dressings and topical agents
- Continually assess and revise the PT care plan
- Educate and instruct clients, family members, or other representatives on rehabilitative care and activities needed to promote health, safety, and independent living
- Accurately document observations, interventions, and evaluations pertaining to patient care using a state‑of‑the‑art touch‑pad tablet
Competitive pay rates with a weekly pay schedule, flexible scheduling. Up to $30,000 bonus available for full‑time positions in specific territories. PTO and paid holidays. Mileage reimbursement or schedule‑specific benefits.
