* Job Description:
** - Utilize the nursing process to assess patient and caregiver needs for quality care.
- Develop and communicate realistic and individualized plans of care in collaboration with patients and caregivers.
- Act as a Case Manager for assigned patients, managing caseloads and visit schedules efficiently.
- Complete OASIS documentation accurately and in a timely manner, staying updated on requirements.
- Implement medical plans of care and nursing interventions, ensuring patient safety and quality care.
- Maintain proficiency in High-Tech skills, including IV use, wound VAC, and complex wound care.
- Collaborate with LPNs to follow care plans, delegate tasks, and ensure patient needs are met.
- Evaluate the effectiveness of care plans, adjusting as needed to meet patient and caregiver requirements.
- Maintain accurate documentation to support ongoing quality of care.
- Follow agency protocols for visit documentation and completion processes.
- Educate patients and caregivers on medication, treatment options, and discharge instructions.
- Identify interdisciplinary needs and communicate them appropriately to other team members.
- Complete necessary agency, state, and federal forms in a timely manner.
- Conduct visits efficiently and communicate visit timing to schedulers.
- Assess patient needs for HCA services and coordinate care plans accordingly.
- Report patient incidents promptly to the QI Specialist.
- Coordinate safe patient discharges or transfers, ensuring necessary paperwork is completed.
- Collaborate with physicians, providers, and team members for effective patient care.
- Participate in monthly peer review audits, providing and receiving constructive feedback.
- Schedule and facilitate case conferences to enhance communication with the team.
- Advocate for patient care issues, ensuring sensitivity to individual patient and family needs.
- Actively participate in IDT meetings to create and update individualized plans of care.
- Stay updated on pain control and symptom management techniques.
