Licensed Practical Nurse (LPN) – Long-Term Care travel assignment in Rutland, Vermont. Start date June 22, 2026 for an assignment measured in weeks with 40 guaranteed hours per week on a consistent 2:00 PM–10:00 PM shift. • Provide direct, hands‑on nursing care to residents in a long‑term care setting, consistent with individualized care plans.
• Administer medications and treatments as prescribed, monitor effects, and document responses in the chart.
• Monitor vital signs, assess changes in resident condition, and report concerns promptly to the supervising RN or physician.
• Assist residents with activities of daily living, including ADLs, mobility, nutrition, and personal hygiene.
• Maintain accurate documentation, ensure infection control practices, and uphold safety and resident‑rights standards.
• Collaborate with an interdisciplinary team to support care planning, patient comfort, and timely changes in care as needed.

• Active LPN license in good standing.
• Graduate of an accredited practical nursing program.
• Preference for experience in a long‑term care or skilled nursing setting.
• Availability to work the 14:00–22:00 shift and to fulfill guaranteed weekly hours.
• Strong communication and documentation skills; ability to collaborate with a diverse care team.
