Registered Nurse – Home Health

Schedule:

8am to 4pm, Monday - Friday, with some on-call and weekend requirements.

Requirements:

• OASIS experience required.
• Current nursing license.
• Knowledge of state and federal regulations.
• Proficiency in high-tech nursing skills (IV use and maintenance, complex wound care, etc.).

Roles & Responsibilities:

• Assess and address patient/caregiver needs using the nursing process to provide quality care.
• Develop and communicate a realistic plan of care in collaboration with patients/caregivers, adhering to evidence-based practices and regulations.
• Serve as the Case Manager for assigned patients, actively managing case loads and visit times.
• Complete OASIS documentation accurately and timely in accordance with agency policies and regulations.
• Implement nursing interventions and procedures to ensure quality and safe care.
• Evaluate the effectiveness of care plans and make adjustments as necessary.
• Coordinate care with interdisciplinary teams to ensure comprehensive patient support.
• Educate patients and caregivers on treatment options, medication side effects, and discharge instructions.

Compliance / Notes:

• Participate in monthly peer review audits.
• Follow agency protocols for documentation and communication with scheduling.
