A leading community-based healthcare organization in Vermont is seeking an experienced Psychiatric Nurse Practitioner to provide outpatient psychiatric care for adolescents within a collaborative, integrated behavioral health program.

• Provide outpatient psychiatric care for adolescent
  patients
• Diagnose, treat, and manage a variety of behavioral health conditions
• Develop individualized treatment plans focused on patient goals
• Collaborate with therapists, case managers, physicians, and support staff
• Perform psychiatric evaluations, medication management, and follow-up care
• Review diagnostic information and monitor treatment progress
• Provide patient and family education on medications, wellness, and mental health strategies
• Coordinate referrals and continuity of care
• Utilize an integrated electronic medical record system

• Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
• Active state licensure as an Advanced Practice Registered Nurse
• 1–2 years of psychiatric or behavioral health experience preferred
• Strong assessment, diagnostic, and clinical decision-making skills
• Excellent communication and interpersonal abilities
• Passion for working with adolescents and families
• Commitment to collaborative, community-based care

• Salary: $130K-$170K
