American Traveler is hiring a Home Care RN with OASIS experience and a VT or compact RN license for a home health hospice position requiring some on-call and holiday coverage.
Details- Home health setting through a hospice and home care agency
- Serves as case manager for an assigned caseload of home care patients
- Shift options of 8am to 4pm Tuesday through Saturday or Sunday through Thursday
- Some on-call responsibilities required
- Holiday coverage required
- 13-week assignment with 37 guaranteed hours
- Active VT or compact RN license required
- Minimum 2 years of RN experience required
- OASIS documentation experience required
- Experience with high-tech nursing skills such as IV therapy, wound VAC, and complex wound care
- Conducts patient and caregiver assessments and develops individualized plans of care in accordance with evidence-based practice and state/federal regulations
- Completes OASIS documentation accurately and in a timely manner
- Implements nursing interventions and coordinates high-tech skills such as IV maintenance and complex wound care
- Collaborates with LPNs, delegating tasks and reviewing documentation to ensure quality care
- Provides patient and caregiver education on medications, treatment options, and discharge instructions
- Coordinates interdisciplinary care needs, including physical therapy and social work referrals
- Communicates with physicians, providers, and team members to support safe and effective patient care
- Participates in monthly peer review audits and case conferences with the care team
- Coordinates safe patient discharges and transfers in accordance with agency and regulatory guidelines
