LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse / Licensed Practical Nurse

Providing professional nursing care to residents in long-term care or skilled nursing facilities, ensuring quality patient outcomes. The LVN / LPN will support daily operations while adhering to facility protocols and safety standards.

Responsibilities:

• Administer medication and treatments accurately following prescribed orders.
• Monitor residents' health status and report changes to the nursing team.
• Document patient care and record health information in compliance with regulations.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Maintain a safe, clean, and comfortable environment for residents.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Minimum of 1 year experience in LTC or SNF settings.
• Demonstrate strong clinical skills and attention to detail.
• Possess reliable transportation and ability to commit to every weekend shifts.
• Be fully vaccinated against COVID-19 or have approved medical/religious exemptions.
