LVN ​/ LPN – LTC​/SNF

Licensed Practical Nurse

Provide licensed practical nursing care to residents in a long-term care or skilled nursing facility setting. Deliver compassionate, patient-centered services to support resident health and well-being while adhering to facility policies and regulations.

Responsibilities:

• Administer medications and treatments accurately and timely.
• Monitor residents’ health status and report changes to the healthcare team.
• Document care provided and maintain detailed, compliant records.
• Follow infection control protocols and safety guidelines consistently.
• Assist residents with activities of daily living to promote independence.
• Wear identification and ensure compliance with all facility policies.

Qualifications:

• Hold a valid LVN or LPN license in good standing.
• Possess a minimum of one year of experience in long-term care or SNF settings.
• Have current CPR and first aid certifications.
• Demonstrate strong communication and interpersonal skills.
• Be able to obtain and maintain required vaccinations or exemptions.
• Have reliable transportation and ability to commit to weekend shifts.
