LVN ​/ LPN – LTC​/SNF

Job Opportunity

Provide skilled nursing care to residents in a Long-Term Care or Skilled Nursing Facility environment. Ensure the delivery of high-quality patient care while adhering to facility standards and protocols.

Responsibilities:

• Administer medication and treatments accurately according to physician orders.
• Monitor residents’ health status and document changes or concerns.
• Collaborate with healthcare team members to develop and implement care plans.
• Educate patients and their families on health management and care procedures.
• Maintain compliance with safety and infection control policies.
• Report any emergencies or urgent issues promptly to supervising staff.

Qualifications:

• Valid LVN or LPN license in good standing.
• Proven experience in LTC or SNF settings preferred.
• Ability to effectively assess and monitor patient conditions.
• Strong communication and interpersonal skills.
• Ability to work independently and follow protocols accurately.
• Must have own transportation and reliable means of travel.
