LVN/LPN Position in Long-Term Care/SNF Setting

Responsibilities:

• Administer medications and treatments accurately following care plans.
• Monitor patient conditions and report changes to healthcare team.
• Assist with patient hygiene, mobility, and daily living activities.
• Document patient information in accordance with facility protocols.
• Ensure compliance with safety and infection control standards.

Qualifications:

• Valid LVN or LPN license in the state.
• Proven experience in long-term care or SNF environments preferred.
• Ability to effectively communicate with patients and team members.

• Meet immunization requirements or provide valid exemptions.
