  • Utilize the nursing process to assess patient and caregiver needs and develop a realistic plan of care aligned with evidence-based practice and regulatory standards
  • Serve as Case Manager for assigned patients, actively managing caseloads and visit times in accordance with agency productivity standards
  • Accurately and timely complete OASIS documentation and stay current with changes to OASIS requirements
  • Implement medical plan of care and nursing interventions to ensure safe, effective, high-quality care; maintain infection control standards
  • Work collaboratively with LPNs and other disciplines to ensure plan of care adherence, delegating tasks as appropriate; document visit notes consistent with policy
  • Provide patient and caregiver education, coordinate care with physicians and team members, and ensure required forms (e.g., HHABN, Medicare Notice of Non-Coverage) are completed in a timely manner
  • Coordinate safe patient discharges and transfers, and communicate incidents to the appropriate stakeholders for quality improvement
  • Engage in multidisciplinary coordination (therapy, social work, etc.) and participate in peer reviews, case conferences, and performance improvement activities as requested

Responsibilities

Requirements
  • Active RN license in Vermont (or ability to obtain) and in good standing
  • OASIS experience required
  • Prior home health or hospice experience preferred; travel assignment experience a plus
  • Strong clinical documentation skills and proficiency with electronic health records; ability to complete required agency and regulatory forms accurately
  • Ability to work in a home health setting with adherence to state and federal regulations; on-call and holiday coverage as required
  • Availability for the schedule described (8:00 am–4:00 pm with Tuesday–Saturday or Sunday–Thursday patterns) and capacity to manage a caseload within assigned visits
