Job Summary:
Seeking a skilled Registered Nurse with OASIS experience to provide comprehensive hospice care. The role focuses on assessing patient needs, developing individualized care plans, and ensuring compliance with regulatory standards to deliver high-quality, compassionate care.
Responsibilities:
- Conduct thorough patient assessments utilizing the nursing process to identify care needs in accordance with evidence-based guidelines and regulatory standards.
- Develop and communicate realistic, patient-centered care plans, involving patients and caregivers in the process.
- Serve as Case Manager for assigned patients, actively managing caseloads and visit schedules to meet productivity standards.
- Complete accurate OASIS documentation in compliance with agency policies, federal, and state regulations, staying current on documentation updates.
- Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective patient care.
- Maintain proficiency in high-tech skills such as IV therapy, wound VAC, and complex wound care, ensuring infection control standards are upheld.
- Collaborate with LPNs within scope of practice, delegating tasks and reviewing documentation to ensure quality and continuity of care.
- Evaluate the effectiveness of care plans by monitoring patient responses and adjusting interventions as needed.
- Maintain detailed, accurate documentation of nursing activities to support quality improvement and team communication.
- Educate patients and caregivers on treatment options, medication side effects, discharge instructions, and reportable conditions.
- Identify and document multidisciplinary needs, facilitating communication with physical therapy, social work, and other team members.
- Complete necessary forms such as HHABN and Medicare notices timely and accurately, ensuring availability during visits.
- Conduct visits efficiently, coordinating communication with schedulers and avoiding unnecessary combination of visits.
- Assess patient needs for HCA services, develop care plans, and provide supervision according to regulations.
- Report incidents like infections or injuries promptly to Quality Improvement specialists.
- Coordinate safe discharges or transfers, ensuring proper documentation and regulatory compliance.
- Facilitate care coordination with physicians, providers, and vendors to support the approved care plan.
- Participate in monthly peer review audits and provide constructive feedback to enhance team performance.
- Schedule and lead case conferences to promote effective communication and documentation.
- Advocate proactively for patient care issues to ensure responsive, personalized care.
Qualifications:
- Valid RN license with current licensure in the state.
- Demonstrated experience with OASIS documentation and hospice care.
- Proficiency in high-tech skills including IV therapy, wound vac, and complex wound management.
- Strong understanding of state and federal regulations, including Medicare and Medicaid guidelines.
- Excellent assessment, communication, and interdisciplinary collaboration skills.
- Ability to manage time efficiently and maintain detailed, accurate documentation.
