LVN​/LPN – LTC​/SNF

Job Summary:

This LVN/LPN position requires dedication to weekend shifts within a long-term care or skilled nursing facility setting. The role involves providing high-quality resident care while adhering to facility and regulatory standards.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor and document residents' vital signs, conditions, and responses to treatments.
• Collaborate with interdisciplinary teams to develop and update individualized care plans.
• Ensure compliance with all safety protocols, including COVID-19 vaccination and exemption documentation.
• Maintain professional appearance by wearing an issued photo badge at all times.
• Transport residents safely and effectively, utilizing personal transportation as public transit is limited.

Qualifications:

• Valid state licensure as an LVN or LPN.
• Proven experience in long-term care or skilled nursing environments.
• Ability to work weekends consistently, with excellent time management skills.
• Must have own reliable transportation.
• Knowledge of infection control procedures, including COVID-19 vaccination requirements.
• Strong communication skills to effectively collaborate with team members and residents.
