LVN​/LPN – LTC​/SNF

Job Summary:

Provide professional nursing care to patients in a long-term care or skilled nursing facility setting. Administer medications, monitor patient conditions, and collaborate with healthcare teams to ensure quality patient outcomes.

Responsibilities:

• Administer prescribed medications and treatments in accordance with facility protocols.
• Monitor and document patient vital signs, symptoms, and progress.
• Collaborate with multidisciplinary teams to develop and update care plans.
• Educate patients and their families on health management and post-discharge care.
• Maintain accurate and timely documentation of patient care activities.
• Follow infection control procedures and adhere to safety standards.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in long-term care or skilled nursing facilities.
• Ability to accurately assess patient conditions and respond appropriately.
• Strong communication and interpersonal skills.
• Knowledge of infection control, safety protocols, and regulatory guidelines.
• CPR and BLS certification required.
