LVN​/LPN – LTC​/SNF

Job Summary:

Provide licensed nursing care to residents in a skilled nursing or long-term care facility. Ensure the delivery of quality patient care in a safe and compassionate environment.

Responsibilities:

• Administer prescribed medications and monitor patient responses.
• Document patient assessments, care provided, and changes in condition accurately.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Maintain compliance with facility policies, safety standards, and infection control protocols.
• Respond promptly to resident needs and emergency situations.
• Ensure all documentation and charting meet regulatory requirements.

Qualifications:

• Valid LVN or LPN license with active state licensure.
• Minimum of 1 year of nursing home or skilled nursing facility experience.
• Ability to lift, move, and position patients safely.
• Strong communication and interpersonal skills.
• Knowledge of healthcare regulations and documentation procedures.
• Must meet facility vaccination requirements and carry proper identification.
