Street Outreach Team Lead
Community Care Network is looking for a Street Outreach Team Lead to join our team! Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. About the Role:
The Street Outreach Team Lead provides day-to-day leadership, coordination, and support to a team dedicated to engaging individuals who are unhoused and/or experiencing substance use challenges. This role works collaboratively with community partners and serves as a key liaison for initiatives such as the Outreach Care Team, Situation Table, and Community Resource Team.
The Street Outreach Team Lead fosters a rapid-response, team-based approach to outreach, ensuring timely engagement and support for individuals when they are ready to access services. Grounded in the principles of person-centered care, harm reduction, and overdose prevention, the position promotes these approaches through direct service, community education, consultation, and role modeling.
As part of Rutland Mental Health's Adult Services Department, the Street Outreach Team Lead maintains strong connections to the broader team through regular supervision, team meetings, training opportunities, and shared office space. While based within Adult Services, this position is primarily community-focused, meeting individuals where they are and providing support in a variety of community settings to reduce barriers to care and increase access to resources and services.

Responsibilities:
- Provide day-to-day leadership, guidance, coordination, and support to Street Outreach Team staff.
- Supervise and facilitate outreach activities focused on engaging individuals who are unhoused and/or experiencing substance use challenges.
- Address individuals needs based on Maslow's Hierarchy of needs, and individual's identified goals.
- Coordinate a rapid-response, team-based outreach model to ensure timely engagement when individuals are ready to access support and services.
- Serve as the primary liaison with community partners and collaborative initiatives, including the Outreach Care Team, Situation Table, and Community Resource Team.
- Build and maintain effective working relationships with community agencies, service providers, and stakeholders.
- Promote and model person-centered, trauma-informed, harm reduction, and overdose prevention practices.
- Provide community education, consultation, and technical assistance related to outreach, substance use, overdose prevention, and engagement strategies.
- Support staff in developing individualized engagement and support plans that address participant needs, strengths, and goals.
- Meet individuals in community settings and provide direct outreach, assessment, engagement, and service coordination as needed.
- Assist individuals in connecting and accessing behavioral health, medical, housing, substance use treatment, and other community resources. Including residential placement, open access, MAT provider, detox, discharge planning/step down from residential to outpatient, crisis, shelter, hotel voucher program, etc.
- Monitor outreach activities, service delivery, and program outcomes to ensure quality and effectiveness.
- Facilitate team communication, case consultation, and problem-solving to support coordinated service delivery.
