What You'll DoAs a Nurse Practitioner, you’ll provide direct, compassionate care to patients receiving hospice and palliative services in their homes, community settings, and hospice house environments.
You will:
Perform comprehensive assessments and provide ongoing clinical management for patients across care settings
Manage pain and complex symptoms using evidence-based palliative care practices
Collaborate closely with nurses, social workers, chaplains, and physicians to deliver coordinated, patient-centered care
Lead goals-of-care conversations and support patients and families through serious illness and end-of-life decision-making
Provide timely visits and clinical interventions to ensure comfort, dignity, and quality of life Document care accurately within the EMRParticipate in interdisciplinary team meetings and contribute to individualized care planning
Foster a calm, supportive presence for patients and their loved ones in any setting
What You’ll Bring Master’s degree in Nursing and NP certification (AANP, ANCC, or equivalent)
Active Maine APRN license
Hospice or palliative care experience strongly preferred
Comfort managing end-of-life care in a residential, community or inpatient hospice setting
Strong clinical judgment and ability to work both independently and collaboratively
Excellent communication skills with patients, families, and care teams
Commitment to compassionate, patient-centered care
ACHPN certification (or willingness to obtain within 2 years) preferred
