As part of Primary Health Services, Adult Correctional Health (ACH) provides a high level of primary care services to the adult inmate population in the Sacramento County Jails. The Sacramento Sheriff’s Office (SSO) has overall responsibility and management for the County Jails. The Department of Health Services (DHS), Primary Health Division provides healthcare services (physical health and behavioral health) through County and contracted staff working in partnership with the Sacramento Sherriff’s Office (SSO).
Services are provided onsite and offsite by County staff and contracted Providers. They include:
Physical Health, Mental Health, and Alcohol & Drug Services.
The Assistant Medical Director D/CF (AMD) provides oversight of medical services in collaboration with the Medical Director D/CF and provides onsite supervision to Providers assigned. The AMD is responsible for a minimum of 30% direct services to patients with awareness of the specific needs of the population being served.
Management Expectations- Provide on-site oversight and supervision of Nurse Practitioners and Physicians (Providers) assigned.
- Work directly with assigned Providers to educate them on Correctional health processes and determine best course of action to provide and improve overall quality of care.
- Collaborate with ACH Administration, including Case Management to support patient access to specialty care providers, non-County Providers, and non-medical facilities.
- Collaborate with nursing to prioritize daily Provider Sick Call appointments for patients based on level of acuity.
- Monitor Chronic Care Program protocols, including Medicated Assistant Treatment (MAT).
- Create and facilitate work groups and committees designed to improve patient care.
- Collaborate with the Medical Director to develop, update, or replace medical County policies.
- Manage Provider Sick Call appointments and Provider visits.
- Communicate with Medical Director to make certain the goals and objectives of the program are met – including consistent processes between the Main Jail and RCCC.
- Develop and monitor projects assigned by the Medical Director and Health Services Administrator.
- Supervise Provider staff assigned.
- Evaluate assigned Provider staff’s quality of work by completing reviews of provider chart notes and treatment plans.
- Assess, train and monitor new Provider staff assigned.
- Meet with provider staff to address issues in performance and/or attendance.
- Maintain an active professional license and certifications.
- Attend classes or workshops to improve medical skills.
- Exhibit excellent customer service, focused on patient care.
- Create a cooperative working environment among peers and ACH personnel.
- Learn and utilize ACH’s EHR, required for documenting patient care.
- Be proactive versus reactive.
- Use good judgment in decision-making.
- Foster a team culture and accept various assignments as needed.
- Work cooperatively with Sacramento Sheriff’s Office personnel.
- Maintain flexibility and adapt to different situations based on the correctional working environment.
- Adhere to quality improvement, safety, environmental, and infection control standards.
