* Evaluate and treat patients in a skilled nursing/post-acute environment
* Develop and implement individualized treatment plans
* Complete evaluations, progress notes, and discharge documentation accurately and on time
* Collaborate with nursing, therapy, and interdisciplinary teams
* Educate patients and families on therapy goals and home exercise programs### ### Physical Therapist (PT) Requirements
* Active
** California Physical Therapist license** (or pending)
* Skilled nursing or post-acute experience preferred, but
** not required
*** Strong communication, documentation, and time-management skills
* Team-oriented, compassionate, and patient-centered approach
