Setting: Home Health (field-based; patient visits in the home/community)

Schedule:
Full-time or near full-time; primarily weekday daytime with flexibility for patient needs
On-Call/Weekends:
No on-call or weekend rotation for therapy clinicians

Finale EMR (mobile point-of-care documentation)
The Physical Therapist (PT) provides skilled physical therapy services to patients in their homes, helping them maximize safety, mobility, and independence. In alignment with the agency's mission to deliver high-quality home health services to all clients regardless of socioeconomic status, the PT delivers patient-centered care that brings the care patients deserve without going far.
The PT collaborates with nursing and the interdisciplinary team to deliver effective plans of care, support positive patient outcomes, and ensure timely documentation in Finale EMR.
Primary Responsibilities- Complete in-home physical therapy evaluations (mobility, gait, balance, strength, endurance, functional assessments) and establish measurable goals
- Develop, implement, and update plans of care consistent with physician orders, payer requirements, and patient-centered goals
- Provide skilled PT interventions in the home (therapeutic exercise, neuromuscular re-education, gait and balance training, transfer training, manual techniques as appropriate)
- Educate patients and caregivers on home exercise programs, fall prevention, safe mobility/transfer techniques, and assistive device use
- Assess home environments and recommend safety strategies, DME, and modifications to reduce fall risk
- Coordinate with nursing, OT, SLP, physicians, and the interdisciplinary team
- Utilize standardized outcome measures and reassessments; adjust plans of care as needed
- Identify clinical red flags and communicate changes promptly per agency protocol
- Complete timely, accurate documentation in Finale EMR
- Supervise PTAs and support personnel per California law and agency policy
- Manage field schedule, caseload priorities, and visit coordination
- Active California Physical Therapist license in good standing
- Successful completion of required licensure examinations (as applicable)
- Current BLS/CPR certification (or obtained within 30 days)
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Home health experience preferred
- Strong independent clinical reasoning and documentation skills
- Comfort educating patients and caregivers in the home
- Proficiency with mobile documentation systems
- Valid California driver’s license and reliable transportation
- Medicare home health experience preferred
- Finale EMR or similar home health EMR experience preferred
- Finale EMR (preferred) or similar home health EMR
- Mobile point-of-care documentation
- Field-based schedule and communication tools
- Support from interdisciplinary team and clinical leadership
- Safe, patient-centered home care delivery
- Timely and compliant documentation
- Strong collaboration with nursing and the interdisciplinary team
- Maintain patient confidentiality and HIPAA compliance
- Follow infection prevention, safety, and emergency procedures
Comply with all federal, state, and local laws and agency policies - Subject to background checks, health screenings, and immunization requirements as applicable
