MSW Case Manager

Overview

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

We want to hire Master prepared Social Workers who will work with a Care Management Team (CMT) consisting of Master’s or Bachelor  prepared Social Worker and Registered Nurse team to provide comprehensive case management services to target population.

You are a Master  Prepared Social Worker (BSW) passionate about making a difference in the lives of individuals.

You are detail oriented, computer literate, and want to work as part of a management team supervising the care of clients who are receiving home and community-based services.

You are good at problem solving, knowledgeable about community resources, and enjoy helping clients solve social determinants of health care problems.

You want a workplace that values the significant differences Social Workers can make to the health and welfare of clients.

You like working 8am-5pm on weekdays with no weekend work.

• Assists the team with finding community resources (i.e., housing; medical equipment and supplies; primary care physician and specialists; essential items like food, clothing, and household goods; medical transportation; and caregivers, etc.) needed to meet the participant7s needs.
• Engages in advocacy efforts to ensure participants receive timely medical services to remain safe in the community
• Responds timely to questions and enquiries from CMT, participants, and other entities for smooth patient care coordination
• Provides education to participants so they can better understand the benefits of services and community supports.
• Assists participants with applications (i.e., Medi-Cal; IHSS; Calfresh; and Regional Center, etc.). Explains eligibility requirements, coordinates transportation for participants and other basic needs of support
• Document all consumer interaction and maintain timely reporting and documentation requirements
• Observes participants behavior; reports significant changes to supervisor or team; documents appropriately
• Daily case conference with CMT about shared participants
• Performs other related duties as assigned.

• Master  degree in Social Work or another related field
• Skilled in operating a personal computer utilizing a variety of standard software
• Valid California Drivericense and eligible to be insured under our liability policy with a clean DMV report
• Your own vehicle and proof of current auto insurance
• CPR certificate required within 30 days of hire

• Knowledge of basic problems and needs of people diagnosed with a physical disability or emotional disturbance
• Knowledge of community and supportive services
• Skilled in assessing and prioritizing multiple tasks, projects, and demands
• Skilled in working within deadlines to complete projects and assignments
• Skilled in establishing and maintaining effective working relations with co-workers and other community providers

• Experience with electronic medical records

This is not a remote role, and you are required to work from our Sacramento office.

You will need your own car and be able to travel to client’s homes in Sacramento County and the greater surrounding area. Work is performed in a community and office environment.

PPE is provided to all employees.

Transporting clients with personal vehicle is prohibited

The full-time role is 40 hours per week, from Monday through Friday from 8am to 5pm with an hour for lunch. No weekend work.

Currently 20% of the time is related to direct home visits, 80% of the time is telehealth video visits and or calls and documentation in the office.
