What You Will Do to Change Lives
The Community Health Worker assists in providing outreach and engagement with eligible Managed Care Plan members and supporting care management through transportation assistance to assigned appointments and social services.
Shifts AvailableFull Time; 8:30 am - 5:00 pm Monday - Friday
Expected starting wage range is $21.00 - $23.24. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)- High School Diploma or GED required
- Two (2) years' experience providing mental health or intensive case management services and coordination
- Paraprofessional with or without lived mental health experience
- Experience supporting individuals with physical health challenges to navigate the physical health care system
- Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
- Willingness to use the company vehicle to drive clients to appointments and groups, etc.
- Willingness to use personal vehicle to drive clients to appointments, clinic, etc.
- Willingness to use personal vehicle to drive to program trainings, etc.
- Paid Time Off:
For Full Time Employee it is 16.7 days in your first year - Nine Paid Holidays & Shift differentials for hourly staff (6% for PM Shift, 10% for Overnight Shift). Weekend Shift differentials for hourly staff (5% for Weekend AM Shift, 11% for Weekend PM Shift, 15% for Weekend Overnight Shift)
