Case Manager

Reports To: Director of Clinical Services

Schedule: Monday - Friday (full-time)

Pay Range: $27.15 – $32.52 (Hourly, non‑exempt)

In collaboration with Inpatient Therapists, the Case Manager provides quality case management and discharge planning services to all patients and their families. The role is a member of an interdisciplinary team that supports the organization’s treatment program and philosophy, ensuring quality treatment delivery to patients and families. Responsibilities include linking patients to mental‑health and recovery services, facilitating communication with County Liaison, arranging follow‑up appointments, coordinating pharmacy referrals, identifying support persons, providing placement support, verifying , arranging transportation, completing Continuing Care Plans, and documenting daily placement.

The Case Manager ensures that all required administrative day documentation is completed in the clinical record, including DTF, IDT, T‑Con referrals, Firearms Prohibition Form, MHRS Report, JV220, and 6552.

None beyond those listed in Knowledge and Experience.

• Minimum of a Bachelor's Degree in Psychology, Social Work, or a closely related field of study.
• One year of behavioral health experience, with experience in severely mentally ill and discharge planning preferred.
• Current CPR certification, or coursework to obtain it within the initial orientation period.
• Current HWC (Handle with Care) certification, or coursework to obtain it within the initial orientation period.
• Familiarity with continuum of care and clinical terminology.

• Maintains confidentiality of patients at all times.
• Sensitivity to and willingness to interact with persons of diverse social, cultural, economic, and educational backgrounds.
• Proficiency with Microsoft Office applications including Outlook, Word, Excel, and PowerPoint.
• Strong organizational skills with the ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.
• Strong written and verbal communication skills.
• Ability to work independently and as part of a team.
• Good judgment, problem‑solving, and decision‑making skills.

• Use standard office equipment; input, retrieve information from a computer; use a keyboard with manual finger dexterity and wrist‑finger speed capable of repetitive actions efficiently for extended periods.
