Position Summary

The Case Manager supports the program by providing case management services to unhoused clients recently discharged from the hospital. The ultimate goal of all services provided is assisting the client with obtaining an income, insurance and housing. These services include assistance with co-occurring disorders (mental health and substance abuse) with the expectation of improving current functioning level and skills acquisition through assessment, engagement, assisting with access to treatment including medical care with a primary care provider, rehabilitation and goals.

The Case Manager will complete intakes of all clients, document progress notes and maintain client records in case files as well as completing service delivery logs. Additionally, the Case Manager will transport clients and coordinate linkage to various community resource programs.

Required Qualifications

  • H.S. diploma or equivalent; 1 year of experience in case management and/or lived experience utilizing Medicaid or other social service programs required. Bachelor's degree preferred.
  • Must be able to work collaboratively with others in a team environment, respecting and valuing the perspectives and experiences of others;
  • Familiar with Harm Reduction, Motivational Interviewing, Client Centered theory;
  • Able to maintain and understand appropriate roles, and boundaries among clients, peers, Salvation Army staff and Wellspace partners/stakeholders;
  • Able to function effectively in a fast paced environment;
  • Ability to write progress notes and maintain detailed, statistical records;
  • Must demonstrate an ability to work with traditionally challenging, diverse populations;
  • Able to rotate after hours on-call pager duties for a week at a time including weekends;
  • Must demonstrate a high level of verbal, writing and listening skills;
  • Must demonstrate a commitment to HIPPA rules of confidentiality;
  • Computer literacy including use of email, Word and Excel;
  • Must be willing to transport clients in company or personal.
  • Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
  • Current CPR and first aid certificate or willingness to be trained required within first (90) ninety days of employment.
  • Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
  • Assist and support the Center/Department/Program to meet standards of High Reliability.
  • Must have valid CDL and insurable driving record.
  • Must be willing to use own vehicle to transport clients daily.
  • Meet with each client individually and conduct an intake needs assessment.
  • Represents and/or advocates for and with clients in obtaining identification, housing, benefits services, and entitlements from governmental and private agencies and organizations.
  • Coordinate linkage to various community resource programs and supports.
  • Collaborate with the team in the development of an individual case plan for each client.
