Job Summary About our Program

Pacific Clinics will be opening a high‑intensity, community‑based Adult Full Service Partnership (FSP) programs for adults age 18+ with serious mental illness (SMI) and complex needs. The program delivers “whatever‑it‑takes,” recovery‑oriented services primarily in the community to stabilize individuals, reduce hospitalizations and incarceration, prevent homelessness, and support long‑term recovery and community integration. The ACT FSP (Assertive Community Treatment - Full-Service Partnership) program is an intensive, community‑based mental health service for 96 individuals with serious mental illness who have high needs.

It provides wraparound, multidisciplinary support, including psychiatry, therapy, case management, crisis intervention, housing assistance, and rehabilitation, delivered with a community-driven approach. The goal is to reduce hospitalization and incarceration, promote recovery, support housing stability, and improve quality of life through continuous, personalized care. This program will also serve those who are involved in the justice system as well. Under general supervision, implements the full range of substance use services within the context of the Certified Community Behavior Health Clinic in community, school, and clinic-based settings.

Responsibilities
  • Outreach and screening
    • Participate and support universal outreach activities to increase awareness of substance use among the target population.
    • Conduct presentations to various target audiences (e.g., schools, community) to increase awareness of the impact of substance use.
    • Conduct substance use screenings and refer to treatment accordingly.
    • Support the Drop-In Center and Youth Development:
    • Receives, reviews, and processes referrals as necessary.
    • Provides referral sources for individuals identified as needing supports through screenings.
  • Substance use treatment
    • Implement client intake, screening, and assessment procedures.
    • Implement and ensure follow through on each client's personalized treatment plan.
    • Ensure treatment plans include crisis intervention and community supports.
    • Provide evidence‑based individual and/or group outpatient substance use treatment in clinic, community, or school based settings.
    • Coordinate care with other service providers and peer supports.
    • Support facilitation of services within Uplift Family Services programs, particularly with the CCBHC, and between Uplift Family Services/CCBHC and external partners (e.g., schools, agencies with MOUs, etc.).
  • Recovery Supports
    • Support Clients in developing WRAP plan and in recovery support efforts.
  • Administration
    • Works with Program Manager on fund development.
    • Participates in internal and external multidisciplinary/cross functional teams.
    • Attend organization and departmental staff meetings as required.
    • Coordinate with evaluation team on evaluation requirements.
