As a Nurse Practitioner Lead at Greater Good Health, you'll be at the heart of our mission to provide exceptional care to our senior Medicare patients. We want our clinicians to take the time needed to address the patient's needs and concerns. This position will be two fold: provide direct oversight to Nurse Practitioners in their market, and perform clinical care.
The Nurse Practitioner Lead will be responsible for the coordination and oversight of all medical care provided and quality of services in their respective markets. He/She is responsible for promoting high standards of practice that could include but not limited to development of policies, protocols, practice guidelines, training, Clinician experience, patient experience, and by participating in rigorous performance improvement monitoring of Clinicians.
Your responsibilities will include:
- Serve as the NP Lead for a team of clinicians. Provides oversight and in the field supervision and coaching including the development of clinicians, evaluating clinical quality, and completion of chart reviews.
- Assist in the interviewing and hiring of new team members and oversees onboarding of new clinicians.
- Creates a work environment in which people are able to perform to the best of their abilities. Constantly communicates to the team, is open to opinions and feedback from team members and follows through on commitments.
- Manage the longitudinal care of an assigned patient panel of CKD and ESKD patients, focused on managing their conditions, timely post‑discharge follow‑up, utilization management, preventing readmissions, and medication management.
- Performs Comprehensive Health Assessments and Health Risk Assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific clinical coding and documentation.
- Provides clear direction to achieve goals, creating an environment that fosters team commitment and employee engagement.
- Ensures NPs complete documentation timely and accurately including appropriate diagnoses, closing clinical quality gaps, and accurate coding.
- Assure that the quality and appropriateness of patient care are monitored and evaluated and that appropriate actions based on findings are taken on a consistent basis.
- Conducts shadow visits regularly.
- Leads NP Huddles and meets with the NPs individually during 1:1s.
- Reports quality of care issues promptly to appropriate individuals.
- Works closely with Operations leadership, Clinical Director, and Care Services.
- Function autonomously, and in collaboration with all members of the healthcare team to coordinate and facilitate high quality, cost‑effective care.
- Support highly complex patients, assisting them in navigating a challenging healthcare system, and providing one‑on‑one patient care they need and deserve.
- Provide patient education, create dedicated care plans, and provide clinical guidance to an integrated care team all with the best interest of the patient as the guiding principle.
