Position: Full Time Home Infusion Nurse

Job Summary

An Infusion Nurse I is a licensed registered nurse who provides basic direct patient care to patients in the home or an alternate infusion site, ensuring patient safety and continuity of care under a physician's plan of care. The role is performed under close direct supervision of the designated supervisor.

Responsibilities
  • Initiate, develop, and implement basic nursing plan of care treatments, evaluating patient progress toward goals.
  • Participate in direct patient care, performing treatments, administering medications, and educating patients and families or caregivers.
  • Modify the plan of treatment in response to changing patient status based on physician orders to achieve established or revised patient care goals.
  • Assess patient needs and physical status at each visit through health data access and patient interview.
  • Re‑evaluate patient needs through physical reassessment, response to therapy, and supplemental physician orders.
  • Obtain and clarify physician orders for treatment revisions, inform physicians promptly of significant changes in patient condition, and provide written summaries within supplemental orders.
  • Provide effective and safe teaching using a patient‑centered care approach to help patients and families achieve independence with prescribed therapy and care needs.
  • Communicate effectively and timely with Naven Health’s clinical team, medical providers, patients, and families or caregivers to facilitate continuity of care.
  • Prepare clinical documentation in real‑time during visits and submit to the nursing department, complying with established timelines for billing optimization.
  • Complete all documentation legibly and apply approved abbreviations and documentation error correction practices per Naven Health’s policy.
  • Under close supervision, coordinate discharge planning and prepare discharge summaries with patient instructions, thoroughly reporting patient care needs, progress, and goals when transferring care.
  • Demonstrate compliance with agency operations, Naven Health’s policies and procedures, professional standards, and local, state, federal regulations and accreditation standards.
  • Maximize work efficiency through the use of computers and automation technologies to validate treatment orders and communicate provided care, and follow the assigned visit schedule.
  • Respond promptly and appropriately to patient requests.
  • Initiate emergency procedures as necessary.
  • Accept accountability for own practice through ethical and professional conduct.
  • Follow established programs and practices within policies and procedures that reflect Naven Health’s mission, values, and objectives.
  • Observe legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Naven Health information, including adherence to HIPAA regulations.
  • Speak knowledgeably about Naven Health’s scope of services and effectively instruct patients on related care plans.
  • Attend to and complete required training modules, in‑services, and continuing education to maintain competency and professional licensure for care and management of patients in the home and/or alternate settings.
  • Effectively provide oversight and coordination of paraprofessionals in the home setting and may act as the supervisor in the absence of the supervisor or nurse manager as applicable.
  • Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses for orientation, onboarding, and supervisory activities as assigned.
  • Participate in nursing on‑call responsibilities as applicable.
  • Perform other related duties as directed by the supervisor.
Qualifications
  • Active and unrestricted RN license in the state of practice.
  • Minimum of 0‑2 years of infusion nursing experience.
  • Current CPR certification.
  • Demonstrated competency in patient care standards for safe delivery of services and infusion skill sets applicable to agency programs and service needs.
  • Advanced certification and training as applicable.
  • Required licensure to operate a motor vehicle in the state of practice with access to a vehicle for business travel and proof of liability insurance.
