*** Unit Care Coordination (case management) Certs MUST have experience with and proficient at Midas, Inter Call, Cerner, Curaspan, and Microsoft Office programs. Experience with discharge planning and utilization review. Minimum of 3 years hospital experience and needs acute level care coordination experience Ratios 1:20 patient assignment. Charge Nurse Manager Weekend Requirement Scrub Color Business casual conservative dress, no open toed shoes, and lab coats.
Minimal to no perfume. A clean, professional look. RT 24/7 Yes Pharmacy 24/7 Yes Shifts 8hrs, Prefer minimal time off requests. Can make a 12 hour shift option IF the candidate has a background in ED and is willing to work night shift. No OT. Floating Need to be able to cover all adult areas of the hospital. If no women's health experience, the traveler will not cover this.
Floor specific orientation 4 days webinar on charting. 2 days preceptor as well. Additional Notes Must be a fast learner, adaptable, flexible and be able to hit floor running. Excellent attendance is expected. Must take lunches and breaks. Located in a very underserved area of Sacramento and our staff work through heavily socially-impacted discharges such as R&B (room and board / board and care), B&C placements, conservatorship, under- or uninsured, etc - so a strong acute hospital discharge planning background is preferred.
Looking for someone who has familiarity working with placement agencies, familiar with Medicare/Medi-Cal guidelines as it pertains to SNF and lower level placement, familiar with strategies to discharge individuals who are not heavily resourced, etc.
