In-Home Health - Nurse Practitioner or Physician Assistant; -Plattsburgh, NY

Responsibilities

Provide in‑home health evaluations, conducting a medication and medical history review, a physical evaluation, and additional diagnostic tests as ordered by the health plan (e.g., diabetic eye exams, spirometry).

Use an iPad and an intuitive clinical workflow for each evaluation. You are not required to prescribe medicine, order lab tests, or alter current treatment regimens.

Travel locally and within licensed states as needed and conduct virtual visits as required.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification required.
• Multi‑state licenses preferred.
• Qualified to cover primary locations:
  Plattsburgh and surrounding area.
• NP or PA.

Primary workplace is the private residence of health plan members. Candidates must be able to safely perform duties in homes that may contain household pets, secondhand smoke, or other potential airborne allergens or irritants.

Typical pay range: $95,738.00 – $. Pay depends on experience, education, geography, and other factors.
