This position reports to the Charge Nurse and Director of Staff Development.

• Provide direct care to residents to improve or maintain the resident’s abilities according to the resident’s plan of care.
• Activities of Daily Living (ADLs):
  • Bathing – Assist with transport to tub, adjust water temperature, wash and rinse resident, dry thoroughly, maintain privacy.
  • Dressing – Locate, select, and obtain clothing for resident, assist with snaps, zippers, buttons, communicate clothing needs.
  • Restorative Care – Assist resident to maintain functioning at highest independent level; perform ambulation, range‑of‑motion exercises, use assistive devices, maintain body alignment.
  • Elimination/Toileting – Assist residents to bathroom according to schedule; provide clean bedpan or urinal, clean residents when unable to self‑clean, measure and record output, provide catheter care per procedure.
  • Mobility – Assist residents or provide range‑of‑motion exercises, provide weight‑bearing assistance or physical support for ambulating, push wheelchair or geri‑chair, maintain safe environment.
  • Transfer – Position residents for transfer; use correct transfer technique and equipment; obtain assistance when needed.
  • Eating – Provide set‑up assistance for meal tray, open, pour, unwrap, and cut food; feed individuals requiring total assistance; encourage chewing; clean after meals; serve nourishment and snacks; document and report intake.
• Protect and promote resident rights, dignity, self‑determination; offer and respect resident choices; refer to residents by proper names; handle property with respect; avoid use of pet names.
• Observe and report patterns of decreased social interaction, angry or sad behaviors and refusals of treatment to Unit Charge Nurse.
• Assist with planning, implementation and evaluation of plans of care for assigned residents; report information about conditions, responses to interventions, suggestions for alternative approaches.
• Assist with preparation for state inspections; attend survey training and interact with state surveyors as directed by immediate supervisor.
• Follow all facility policies and procedures including infection control; use personal protective equipment properly.
• Record accurate, legible information about resident care and condition in appropriate sections of resident record.
• Communicate with Charge Nurse about assigned residents regularly throughout shift.
• Demonstrate competence in proper body mechanics; use equipment such as back belts, gait belts, mechanical lifting equipment.
• Monitor physical environment for safety or fire hazards; report potential problems immediately.
• Report injury to resident or self to Charge Nurse immediately.
• Attend 12 hours of in‑service education annually; apply information to job tasks.
• Participate in fire and disaster drills; carry out assigned duties in emergencies.

• Sufficient mobility and strength to move through building, assure resident safety; assist, transfer or otherwise move residents in danger.
