The Yolo Cares Clinical Team

You will join a highly skilled, compassionate, interdisciplinary clinical team dedicated to providing patient- and family-centered hospice care. Yolo Cares’ clinical team includes registered nurses, hospice aides, social workers, spiritual care counselors, nurse practitioners, and physicians who work collaboratively to support patients and families through every stage of the hospice journey. The team is grounded in clinical excellence, mutual respect, and thoughtful coordination, with a shared commitment to dignity, comfort, and individualized care.

About

The Role

In this role, you'll build meaningful relationships with patients, families, and community partners. As an RN - Case Manager, you'll provide compassionate, patient-centered hospice nursing care while coordinating services across the interdisciplinary care team.

Key Responsibilities Clinical Care
  • Complete comprehensive nursing assessments and ongoing patient evaluations. Provide skilled hospice nursing care and symptom management.
  • Assess changing patient needs and develop individualized plans of care.
  • Educate patients and families regarding medications, disease progression, symptom management, and hospice philosophy.
Care Coordination
  • Collaborate with physicians, nurse practitioners, social workers, spiritual care counselors, hospice aides, and volunteers.
  • Develop and maintain collaborative relationships with partner facilities while coordinating patient care and participating in interdisciplinary care conferences.
  • Coordinate medications, durable medical equipment, and medical supplies.
  • Communicate changes in patient condition and obtain appropriate physician orders.
Documentation & Compliance
  • Complete accurate and timely clinical documentation within established standards.
