Weekend Admission Nurse​/Start of Cares (RN) - Home Health

Part time Registered Nurse Case Manager

Anticipated Territory:
Greater Sacramento Area

Schedule:

Every Saturday and Sunday full days; consider Saturday, Sunday plus a 3‑day work week.

HHS require field visits to patients’ homes.

• Conduct Start of Care (SOC) Admissions – OASIS
• Conduct follow‑ups and evaluation visits as needed in the designated territory
• Make home visits to patients in the designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions and report changes to the clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided

• Concierge resources for case managers to assist with administrative tasks
• Flexible work schedules—create your own schedule and hours

• Current and unrestricted California RN license
• Current BLS/CPR certification
• Valid California driver’s license
• Experience conducting OASIS assessments and Start of Care is highly desired
• High level of clinical knowledge in home health care is highly preferred
• Experience with high‑acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case management fundamentals are highly desired
• Previous experience with IV medication infusions is highly preferred
• Proficiency in initiating intravenous (IV) therapy is a plus
• Self‑directed, empathetic, and a critical thinker
• Outstanding interpersonal and leadership skills
• Organized and detail‑oriented
