Licensed Vocational Nurse (LVN) - Home Health Assignment Duration

This is a 13-week contract position.

Monday to Friday, 8-hour shifts (Day Shift).

We are seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to provide high-quality home health care services. The LVN will be responsible for performing routine visits and ensuring that each patient receives proper care according to established protocols.

• Deliver routine nursing care to patients in their homes.
• Ensure care plans are followed and evaluate patient conditions regularly.
• Document patient interactions and progress in the system.
• Assist in coordinating patient care with other healthcare professionals.
• Maintain a safe and clean environment for patients.

• Current LVN licensure in California.
• Experience in home health care required.
• Familiarity with Home Care Home Base (HCHB) preferred; candidates without experience may be considered at a reduced rate for training.
• Must achieve a minimum of 32 points per week through patient visits.
