Registered Nurse (RN) – Home Health (Per Diem)

Sonder Healthcare is seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Sacramento County. This position offers schedule flexibility, autonomy in the field, and the opportunity to make a meaningful impact within the community.
Responsibilities- Perform initial and ongoing patient assessments
- Develop and update individualized plans of care
- Provide skilled nursing care, treatments, and medication administration
- Educate patients and caregivers on disease management and self-care
- Coordinate care with physicians and interdisciplinary team members
- Maintain accurate and timely clinical documentation
- Active California RN license (required)
- Minimum of three (3) years nursing experience
- Home health experience required
- Current BLS certification (American Heart Association)
- Valid driver’s license and reliable, insured transportation
- Strong clinical, documentation, and communication skills
- Flexible per diem scheduling
- Independent field-based role
