Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation. o All HBS specialties require 2 years of current Hospital clinical activity in the specialty.
o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties. ________________________________________________________________ ADDITIONAL DETAILS inpatient/rounding in Cardiology within the last 2 years required. We are looking for one provider to cover a Monday-Friday schedule.
