PRN Physical Therapist (PT) – Home Health

As a Home Health Physical Therapist, you'll develop individualized treatment plans that improve mobility, reduce pain, restore function, and enhance quality of life.
What You'll Do- Perform comprehensive physical therapy evaluations and establish individualized treatment plans.
- Develop and implement therapeutic interventions to improve mobility, strength, balance, and functional independence.
- Monitor patient progress, modify treatment plans as needed, and document outcomes.
- Educate patients, families, and caregivers on exercises, safety, and disease management.
- Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated patient care.
- Maintain timely, accurate, and compliant clinical documentation.
- Follow infection control practices and all agency policies and procedures.
- Promote exceptional patient care while supporting the mission and values of the agency.
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist (PT) license in the state of practice.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- One (1) year of home health experience preferred.
- Home Care Home Base experience preferred.
- Ability to stand, walk, bend, kneel, squat, crawl, and sit for extended periods.
- Ability to lift, carry, push, and pull up to 100 pounds with or without reasonable accommodation.
