POSITION SUMMARY:

Under the general supervision of the Mental Health Supervisor (State Facility & Forensic Intensive Case Management) and/or (Community Support Services), functions as the liaison and coordinates and supports the State of Michigan facilities for individuals with mental illness including the Forensic Center. Develops and monitors service and support plans containing desired outcomes, supports to be provided, methodologies to assist consumers in achieving their desired life outcomes using person-centered planning techniques.

Facilitates consumer integration within their community. This position will be knowledgeable about and actively support culturally competent recovery-based practices; person-centered planning as a shared decision-making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma-informed culture of safety to aid consumers in the recovery process.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Functions as liaison in State of Michigan facilities for individuals with mental illness including the Forensic Center. Prepares professional diagnostic assessment of consumer and participates in personal future planning activities.
  • Works to transition the consumer from inpatient care and/or State Facility to community settings. Assesses whether the consumer meets the criteria for ongoing case management. If the consumer meets the criteria, will transition the consumer to an assigned case manager. If the consumer does not meet the criteria, will provide short-term case management until the consumer is stable and all necessary supports and referrals are in place.
  • Works with consumer to plan, schedule, develop and implement a person-centered plan. Carefully monitors required paperwork for forensic-involved individuals.
  • Assures that all consumers are offered the opportunity to have an independent facilitator facilitate their meeting and that all consumers are made aware of self-determination and offered the opportunity to participate in self-determination.
  • Utilizing the person-centered planning process, develops a support plan containing outcomes, support arrangements, health and safety needs, and methodologies to support the consumer in achieving their desired life outcomes. Coordinates and in-services residential providers on the implementation of plans of service.
  • Provides individual and group treatments utilizing evidence-based practices when appropriate.
  • Monitors level and quality of services provided to consumers in all settings, especially in State of Michigan facilities and includes customer satisfaction with services. Reviews progress notes, writes progress statements and quarterly reports and gives feedback to consumer and caregivers concerning progress.
  • Organizes, facilitates and participates in planning and discharge meetings to develop intervention and supports plans. Prepares meeting notes and takes reports written by other professionals and prepares the service and support plan.
  • Links and coordinates supports and services for consumers to ensure quality of life including finding and maintaining residential placements, clinical services, public service and addressing family needs. Facilitates consumer development of community networks of supports. Provides crisis intervention/short-term counseling as needed.
  • Assists consumers in securing inter-agency resources, including long-term counseling, skill-building services, support employment, substance abuse intervention, etc. Participates in the process for authorization of internal and external services and supports. Actively assists consumers to apply for benefits. Provides information and referral for community resources.
  • Coordinates communication of consumer status with physician offices, crisis, nurses, therapists, psychiatrists, or other professionals. Keeps resident providers, family members, and other professionals informed of changes.
  • Participates in staff meetings and individual supervision to receive current information, guidance, and direction from Supervisor.
  • Facilitates conflict resolution of service and support-related problems.
  • Prepares and maintains case management paperwork such as assessments, periodic reports, case files, progress notes, documentation, financial updates, invoices, etc.
  • Works cooperatively with peer support specialists or other agency staff to meet the needs of consumers.
