Summary

The purpose of this position is to care‑manage patients/residents as assigned, collaborating with and supervising other disciplines and services. Such collaboration must be in accordance with current federal, state and local standards and guidelines to ensure that the highest degree of quality is maintained at all times.

Responsibilities
  • 40% – Responsible for the assessment, development and ongoing changes of patient/resident care plans, and quality outcomes.
  • 25% – Participates in direct patient/resident care as needed.
  • 20% – Ensures appropriate care plans are initiated and updated with condition change, and documentation supports care provided.
  • 15% – Daily rounding with staff and patient/resident.

Required Education

Associates Degree in Nursing required.

Other Information Other Duties and Responsibilities
  • Develops and maintains good working rapport with inter-departmental personnel to meet the needs of each patient/resident.
  • Completes performance appraisals for assigned staff with collaboration of manager.
  • Conducts counseling/disciplinary meetings with staff with collaboration of manager.
  • Addresses and completes appropriate follow through on: patient/resident/family concerns/complaints; staff concerns; variances (including investigation); follows the Policy & Procedure, written guidelines, and standards.
  • Makes recommendations for development and/or revisions.
  • Communicates complaints, problems, and/or concerns to the DON, Chief Nursing Officer, and/or Administrator.
  • Participates in Performance Improvement.
  • Prepares/maintains necessary records and reports.
  • Participates with manager regarding staff scheduling to meet the needs of the facility, assures staffing levels to meet patient/resident needs and meet regulatory standards.
  • Assists with the orientation and ongoing competencies in collaboration with manager.
  • Instructs other team members when appropriate, explains goals and rationale for individualized care.
  • Collaborates in the development, planning and presentation of education activities.
  • Participates in regulatory surveys.
  • Actively participates or leads committees as assigned or appropriate.
  • Requires competency in Microsoft® Windows.
  • May be required to participate in further learning opportunities offered by My Michigan Health.
  • On call after hour and weekend coverage.
  • Participates in activities/programs to be current on regulations, guidelines, and professional standards.
  • Remains current on new developments by attending professional institutions, reading professional journals and attending seminars.
Experience, Training and Skills

Demonstrated leadership abilities; able to work under stressful situations; and displays use of professional verbal and written communication skills with integrity and initiative.
Read, write, speak and understand the English language.
Ability to make independent decisions when circumstances demand.
Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and general public.
Willing to seek out new methods and principles and incorporate them into existing practice.

Physical/Mental Requirements and Typical Working Conditions

Exposure to stressful situations, including those involving public contact, as well as trauma, grief and death.
Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.
