Job Overview

Residential Home Health and Hospice is seeking a Licensed Practical Nurse (LPN) to provide exceptional home health care for patients in Alpena, MI and surrounding areas. The LPN will collaborate with a team-based care environment to manage and deliver patient care plans.

• Support patients in Alpena and surrounding areas.
• Collaborate with the care team to manage and deliver patient care plans.
• Maintain accurate and timely charting, meeting critical deadlines.
• Travel up to 20% of the day to patient locations.
• Utilize tablets, mobile phones, and EMR software for documentation and communication.

• LPN license in the state of employment.
• Graduate from an approved school of practical nursing.
• Minimum two years of LPN experience.
• Valid driver's license; willing to drive approximately 20% of the work day.
• Ability to self manage time and schedule.
• Commitment to meet critical deadlines for charting.
• Experience with tablets, mobile phones, and EMR software.
• Successful completion of a drug screen (including medical and recreational marijuana) and background check required prior to employment.
