Responsibilities:
Provide direct patient care to residents with varying levels of needs, including assisting with activities of daily living (ADLs), administering medications, and maintaining accurate records
Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the unique needs and goals of each resident
Monitor and report changes in resident condition to the nursing staff and other healthcare professionals
Perform routine vital sign checks, wound care, and other medical procedures as needed
Maintain a safe and clean environment for patients, staff, and visitors
Participate in quality improvement initiatives to continually improve the delivery of patient care

Requirements:
Licensed Practical Nurse LPN with a current Michigan LPN license
