- Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment
- Determine follow‑up assessments according to patient needs and best practice guidelines
- Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility, and other personnel involved with patient care
- Assess health status by obtaining relevant health and medical history
- Perform a physical exam based on the age and patient history as per the Standard Care Arrangement with a collaborating physician
- Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient’s clinical problems and healthcare needs
- Perform diagnostic or therapeutic interventions consistent with certification, law, and state medical privileges
- Prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency
- Develop a treatment plan in collaboration with the patient and caregivers
- Refer patients to physicians as appropriate for consultation or for specialized health resources and treatment
- Ensure continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas
- Provide patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits
- Hold case conferences with families when appropriate to advance the care plan
- Meet minimum productivity requirements based on practice standards
- Master’s or doctoral degree as a Board‑Certified Nurse Practitioner or Physician Assistant
- Current State License as a Nurse Practitioner or Physician Assistant
- Sit, stand, bend, lift, and move intermittently
- Must be able to lift at least 35 lbs and bear the weight of an average adult effectively
- Current and valid driver’s license
- Reliable transportation and valid automobile insurance coverage
- Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in skilled nursing facility settings
- Demonstrates skill in practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
- Ability to handle multiple priorities, documentation requirements, and deadlines
- Post‑acute or Nurse Practitioner/Physician Assistant experience preferred
- Adult, Family, or Gerontology certification preferred
- Experience with geriatric patients preferred

Saginaw, MI - Main Service Area:
Saginaw, Bay City, Midland, and surrounding areas- This position services patients in facilities and private home settings
- 8:00 AM – 5:00 PM, Monday through Friday
$100,000—$120,000 USD
